Violência armada e sistemas de saúde: quatro trajetórias históricas de transformação institucional
Resumo
Introdução: Os conflitos armados constituem choques sistémicos que afetam profundamente os sistemas de saúde, comprometendo infraestruturas, força de trabalho, financiamento e governação. Contudo, a história demonstra que os seus efeitos não se limitam à destruição, podendo desencadear processos de inovação institucional, reorganização sistémica e, em certos contextos, de incorporação de capacidades adaptativas reutilizáveis em crises posteriores. Esta leitura exige, porém, cautela: a identificação de inovação em contexto de guerra não deve ser confundida com a valorização da guerra como motor necessário de progresso, uma vez que a violência armada produz danos sanitários diretos e indiretos prolongados.
Objetivo: Analisar comparativamente o impacto de conflitos armados ao longo dos séculos XIX e XX na transformação das ciências da saúde e dos sistemas sanitários, através de quatro momentos históricos estruturantes: as guerras imperiais do Século XIX; as Guerras Mundiais; a Guerra Fria e o período pós-Guerra Fria.
Resultados: Identificam-se quatro padrões estruturais de transformação: inovação técnico-organizacional em contexto pré-sistémico; centralização e consolidação de sistemas nacionais de saúde após guerra total industrial; tecnologização biomédica no quadro da competição bipolar, com efeitos desiguais em contextos coloniais e pós-coloniais; e fragmentação administrativa com dependência externa em cenários de desintegração política. Nestes últimos, embora predomine a descontinuidade institucional, observam-se por vezes traços localizados de antifragilidade, incorporados nas práticas e nos profissionais.
Conclusão: A violência armada não produz automaticamente inovação nem colapso definitivo. O seu impacto depende da coesão política, da capacidade institucional prévia e do enquadramento internacional. Os sistemas de saúde transformam-se menos pela guerra em si do que pelas decisões estruturais adotadas durante e após o choque violento. A consolidação sanitária duradoura depende da reconstrução política, da redução da violência estrutural e do aprofundamento da paz.
